| Terms of Business |

We do not grant payment grace dates for invoices. All invoices become immediately due and payable at the date they are raised. |

By accepting treatment and services, and by signing the new patient or client forms, or by registering via e-mail or web, you hereby undertake to pay Meridian for all costs, expenses and monies incurred in the course of your treatment. |

In the event that your treatment is provided on the understanding that is to be covered by private medical insurance, and, if for any reason whatsoever, payment is refused, delayed (beyond 21 days from invoice) or in any other manner not forthcoming to Meridian, you (the patient or client) become immediately liable for all outstanding sums and undertake to settle these in full within 5 days of notice being given. |

Interest chargeable under the late payment act is due from the date of invoice. No 30-day grace or credit period exists unless this has been previously agreed in writing by Meridian Sports. |

Charges may be applied to your account in the event of late or non-payment. These charges will be incurred where unusual administration costs (letters / phone calls) are needed to recover monies, or in the event that your account is passed to a debt collection agency, further charges will apply. |

Appointments canceled with 24 hours of the appointment time will attract a cancellation charge of £20.00. This charge may vary with time and will be notified to you upon request when booking your appointment. |
| Patients / Clients Responsibilities |
| 1. |
It is your responsibility to disclose to us on your new patient or client form or in writing, or via e-mail or web enquiry, details of any medications, previous injuries or medical history that you have had, in order to ensure best quality care and advice is given. Failure to disclose this information may adversely affect your treatment and Meridian accepts no responsibility from any losses or damages arising from such incidents. |

The length of your appointment / consultation allows time for your practitioner to make proper notes about your condition, coordinate and discuss further appointments you and dispose of any administration associated with your treatment. For this reason it is essential that you turn up on time for your appointment and help us process further appointments and payments quickly at the end of your appointment. |

Meridian operates a 24-hour injury helpline. In the event of any changes in your condition, please call this line at any time of day or night for advice or assistance. |
| 4. |
Please do not use the injury response line for routine appointments. You could be stopping an urgent call getting through from someone who needs our help. |
