The confidentiality of patients, clients and users of Meridian's Services is of paramount importance to Meridian Sports. |

Meridian Sports is registered under and fully complies with the Data Protection Act and all relevant legislation. In accordance with the act you may request a copy of the information we hold on you from Meridian Sports, Bolton Arena, Arena Approach, Horwich, BL6 6LB |

In addition Meridian fully complies with relevant medical and professional body guidelines and statutory legislation regarding patient confidentiality and disclosure of information to third parties. |

Any medical information will be kept confidential. It will only be disclosed to those involved in your treatment and care, or to those persons or organisations responsible for meeting your costs and expenses for such care. |

We do not make nor will we ever allow names and addresses, phone numbers or e-mail addresses of patients, clients or users to be released outside of Meridian Sports, except as part of medical disclosure. |

To comply with best practice your GP will be informed of your assessment result and proposed treatment at Meridian. |
